It is important that you fill out the requested new patient forms before your visit. Allow yourself at least 30 minutes to fill them out. Upon completion, you may mail them to us prior to your appointment or bring them with you when you come.

Plan to arrive 15 minutes prior to your appointment. Doing so will allow sufficient time for the front office staff to process your information and get you in to see the doctor on time.

Your first appointment will last for about 1 1/2 to 2 hours. Subsequent “follow-up” visits usually last around 30 minutes. You will receive confirmation of your follow-up visit prior to your appointment.

Please bring your insurance card(s) and driver’s license with you for every visit. We will scan them before each appointment. This has proven to be the most effective way to follow all the rules that insurance companies require of us.

Also, be aware of the co-pay amount, deductible, or percentage total that your insurance company requires, and be prepared to pay as you check in. We accept cash or the following credit cards: VISA, MasterCard, Discover & American Express.

Please bring all medications you are currently taking.

Finally, due to the amount of time reserved for your visit, should you need to cancel or reschedule, we kindly ask that you call and give us 24 hours' notice of cancellation. This is necessary to avoid a cancellation fee and enables us to schedule someone else for that time.
